Your First Visit to Green Valley Dental

 At your first visit to Green Valley Dental in Monroe, Wisconsin you will be warmly greeted by our professional dental care team. We begin your visit by gathering information to help us get to know you and understand what brings you to our office. That way we can formulate a plan that will help you meet your dental goals while considering your medical and dental history. After you have completed the necessary forms, we will be able to assist you in maximizing your dental benefits.

Moving to our dental treatment area, we begin gathering necessary information to provide use with current diagnostic x-rays and photos of your teeth. At your initial appointment you will receive an examination by the dentist who will then review with you your dental treatment options. At this time we will work together to create a personalized dental care plan to create and maintain healthy, natural, beautiful teeth.

Usually treatment is not started on the initial new patient visit, but if we have adequate information to assess and begin treatment the same day, we will. However, a complex medical history or dental treatment plan will require an evaluation and a second appointment to provide treatment on another day.

When you are scheduling your initial appointment at Green Valley Dental, please share any information with our front desk that may be useful in allowing us to schedule and provide an appropriate amount of time in our dental office for a thorough evaluation of your dental care needs.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.

  • A list of medications you are presently taking.

  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.


IMPORTANT:
 All patients at Green Valley Dental under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-RAYS: If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them with you to your appointment at Green Valley Dental. If additional x-rays are necessary, they can be taken at our facility.
